Time and gender trends in annual incidence show that peak age of diagnosis is between ages of 3 and 4 for both genders (Table 2). There was a 4- to 5-fold increase in annual incidence in more recent birth cohorts among 3-year-olds (0.52 per 1000 in 1998 to 2.32 per 1000 in 2004 for both genders). The patterns of results seen in Table 2 are also shown in Figure 1 for boys and Figure 2 for girls. There was an overall tendency for higher annual incidence among more recent birth cohorts across all age groups.
